Dental Patient Information - Oshkosh, WI
What to Know
When You Join Our Dental Office
We’re so glad that you’re considering Oshkosh Complete Dentistry for you and your family’s oral health needs. It’s a top priority for us that you feel comfortable during your appointments and well-prepared before your very first visit. Below, you can learn about everything you need to get started as a new patient, including expectations for your first visit, forms to fill out, and financing information. If you have any questions or concerns, feel free to let us know!
Your First Dental Visit
As part of your first visit as a new patient, you’ll be asked to fill out essential forms if you have not done so already, then hand them to our front desk team member. Once Dr. Heil is ready to see you, you’ll be led to one of our operatories to complete a comprehensive exam. This includes an examination of your teeth, gums, and other oral structures, and high-resolution photos of your jaw joints in case any issues related to your TMJ are present.
New Patient Forms
As a new patient, we want to make sure that we are tailoring your dental needs specifically to you. That’s why we ask all new patients to fill out a series of detailed forms, which asks them about any oral health concerns they may have, medications they are currently taking, and other useful information. To expedite your appointment, you can download our forms, fill them out ahead of time, and bring them to our dental office.
Dental Insurance Welcome
While we pride ourselves on providing dentistry that is of high-quality, rather than simply what is covered by dental insurance, we are happy to help you maximize your benefits however we can. At this time, we are in-network with Delta Dental and gladly accept most other insurances. If you have questions about your plan, feel free to let us know either at your appointment or by giving us a call.
Ask our friendly team if we take your plan.

CareCredit Financing Available
If you need help covering the costs of your dental treatments, either because you do not have dental insurance or your benefits did not cover all of your costs, third-party financing through CareCredit is a great way to make care affordable. In many cases, you can make monthly, low-to-zero interest payments and complete the treatments you need to protect your long-term oral health. Need guidance on the approval process? Let our front desk know!
